Client Informed Consent and Policies & Procedures

Counselor Credentials: Kelsie McLeod has a Masters degree from Lamar University in Clinical Mental Health Counseling. She is a Licensed Professional Counselor-(LPC), Kelsie is also a National Certified Counselor (NCC) through the National Board of Certified Counselors. In addition to licensing and national certification, Kelsie is a Child & Adolescent Mental Health Specialist through the Child & Adolescent Mental Health Providers Specialty Program through Lamar University. 

Insurance Authorization: Kelsie is in network with Aetna, United Healthcare/Optum, Oscar, Oxford. Insurance approval is to be completed prior to the appointment and is processed through Headway.

Payment: Payment is due at the time services are rendered. There is a $25 charge on all returned checks. 

- Individual session rate is $120 per session. 

Appointment Times: Appointment times are limited. Each session is 50 minutes long. If you are unable to make a scheduled appointment, 24 hours cancellation notice is required or you will be responsible for the full payment of the missed session. A 24 hour cancellation notice is also required for prepaid sessions or the amount will be deducted from the prepaid total. If you are going to be late, please note that only 10 minutes will be allowed or you will have to reschedule and will be charged for that visit.

Refunds: There will be no refunds issued.

Goals of Therapy: Goals of treatment will be developed collaboratively between the client/family and Kelsie. Therapy is a joint effort between the therapist and the client, the results of which cannot be guaranteed. Progress depends on many factors including motivation, effort, and other life circumstances such as interactions with family, friends, and other associates. 

Risk of Treatment: Talking about problems can sometimes make you become uncomfortable, more anxious, and/or depressed. This feeling tends to to decrease as you become more familiar with your therapist. Only a minority feel their conditioned worsened as a result of treatment.

Length of Treatment: The length of treatment will be determined in discussion between you and Kelsie. You may withdraw from treatment at any time. If you elect to stop treatment, Kelsie will provide you with names of other clinicians with whom you may want to continue treatment.

Confidentiality: The information you provide Kelsie is confidential between her and the client and is not released to others without your written consent. She will not fax information to anyone due to the possible risk of others seeing information without your approval. By law, Kelsie is required to disclose confidential information without your consent in the following circumstances: if you disclose the abuse of a minor, elderly, or disabled; if you report to Kelsie that a previous helping professional engaged in a sexual relationship with you; if you file suit against Kelsie for breach of duty; and if a court order or other legal proceeding or statute requires disclosure.

Court Ordered Therapy: Payments must be made in full as mandated by the court. Compliance or non-compliance will be all that will be reported when completed. You must arrive on time for your appointments. If you are late, please note that only 10 minutes will be allowed. If you are more than 10 minutes late, you will have to reschedule and be charged for the visit.
